HOME HEALTH – SPEECH LANGUAGE PATHOLOGIST (SLP)
We have a great opportunity for a PRN or Part-Time Home Health Speech Language Pathologist (SLP).

Service Area – Muncie, Alexandria, Kokomo and surrounding areas
Monitor carefully the patient status and progress toward goals, as well as compliance with physician’s orders. Responsible for educating patients and caregivers, while preparing the patient/caregiver for discharge as appropriate.
Job Responsibilities
- Provide skilled speech language pathology in accordance with the physician’s plan of treatment.
- Observe, record, and report the client’s reaction to treatment and any changes in the client’s condition to the physician, DON/ Clinical Manager, according to policy.
- Develop, implement and revise the client’s plan of care.
- Promote coordination of care through interdisciplinary case conferences and communication with physician and field staff.
- Perform therapy interventions utilizing standard speech language pathology techniques and skills as appropriate for the condition of the patient.
Education, Licensure(s) and Qualifications.
- Minimum of 2 year experience as a Speech Language Pathologist required.
- Previous Home Health experience is preferred.
- Must be currently licensed to practice in the state of Indiana
- Must be CPR certified
- Current Drivers License, vehicle insurance and access to a dependable vehicle
