Registered Nurse (RN) – Adult Day Care Program Overview
We are seeking a compassionate and dependable Registered Nurse (RN) to support participants in an Adult Day Care / Adult Medical Day Program. This role focuses on medication management, health monitoring, and coordination of care in a structured, daytime setting. Ideal for nurses who enjoy a steady schedule, patient interaction, and a lower-acuity environment while still using strong clinical judgment.
Responsibilities
- Conduct nursing assessments, including vital signs, health screenings, and ongoing condition monitoring
- Administer medications and treatments in accordance with physician orders and facility protocols
- Monitor and document participant health status and report changes to the care team
- Maintain accurate nursing documentation using EMR/EHR systems in compliance with HIPAA standards
- Coordinate care with physicians, social workers, therapists, and program staff
- Assist with chronic condition management, including diabetes, hypertension, and dementia-related care
- Provide health education to participants and caregivers as appropriate
- Respond to medical concerns or emergencies and follow established procedures
- Ensure compliance with infection control, safety standards, and state regulations
Requirements
- Active New Jersey Registered Nurse (RN) license
- Experience in adult day care, long-term care, assisted living, home care, or community health preferred
- Strong medication administration and assessment skills
- Experience working with geriatric populations and memory care strongly preferred
- Ability to work independently and as part of a multidisciplinary team
- Comfortable using EMR/EHR systems
- Reliable and professional with strong communication skills
