Inova Health System's Palliative Medicine & Comprehensive Care program is seeking a dedicated Hospice Coordinator to join our dynamic team! The role will be remote or on-site, part-time. Schedule is 2-10s (flexible start/end, between 8:00 a.m. - 6:00 p.m.), Fri & Mon.

Hospice Coordinator Job Responsibilities:
- Explains hospice philosophy, levels of care, goals and services to patients/families, nursing staff and providers to ensure understanding of principles of care.
- Communicates referrals to hospice agencies as indicated.
- Maintains relationships with hospice agency staff to facilitate communication and transitions of care.
- Demonstrates knowledge of community resources to meet patient needs.
- Provides in-services to providers, nursing staff and Case Management as appropriate. Assesses knowledge of staff and provides information on an individual basis when indicated.
- Utilizes standards, guidelines, and pathways for care delivery.
- Identifies trends and develops/implements processes and plans for addressing these trends appropriately.
- Assists with compliance aspects related to hospice.
- Collects appropriate outcome and referral data and reports to leaders on a regular basis.
- Performs other duties as assigned.
Hospice Coordinator Additional Requirements:
- Work schedule: remote or on-site, part-time. 2-10s (flexible start/end, between 8:00 a.m. - 6:00 p.m.), Fri & Mon
- Education: ADN, BSN or MSW. If RN has an Associate's Degree, within 2 years of date of hire, they must meet with their nurse leader and conduct the following: 1.) Identify which accredited school they plan to attend 2.) Provide a written plan with anticipated BSN completion date 3.) Submit a review of transcripts from the school indicating the required pre-requisites and timeline for taking the courses 4.) Complete BSN within 5 years of start date
- Experience: Three years of public health/ community health, medical/ surgical, palliative or hospice nursing/ SW experience. Hospital based nursing/ SW experience.
- Certification: If ADN or BSN, current RN license in VA or eligible to practice in the Commonwealth of Virginia as an RN. If MSW, current SW license in VA or eligible to practice in the Commonwealth of Virginia as a Social Worker. BLS from the American Heart Association.
Hospice Coordinator Preferred Qualifications:
- Experience: 3 years of hospital based MSW or RN experience in hospice
