What are the responsibilities and job description for the Housing Navigator position at Institute for Human Service?

Summary:

The Housing Navigator provides specialized case management and crisis support services for clients staying at the designated program (FTE Hale Mauliola or FTE Housing). Responsibilities include, but are not limited to, development of a housing plan, facilitation of safe, decent and affordable housing options, psychosocial assessment, Behavioral Health assessment, creating recovery plans, providing education and support for the client and others in the client’s significant support network, linking clients to community resources to further independence and stability. Works collaboratively with across teams. Ensures that client input is included in every step of the entire treatment and recovery process with a commitment to work with clients.

Essential Functions:
  • Manages a caseload and oversees the timely coordination of all CM services to include housing solutions/Engage with consumers 1-2 times per week.
  • Supports access to substance abuse assessments and linkage to substance abuse treatment as appropriate.
  • Meet consumer outcomes by determining the objectives needed to reach the client’s recovery goals and identifying service needs that may or may not include contact with individuals, families, guardians, and/or significant others if the client desires
  • Maintains referral network with other human service agencies in the community and makes the appropriate referrals to other programs, community organizations, etc.
  • Make collaborative outreach attempts, including transportation of consumers as needed, to other involved or needed providers to coordinate member’s care as necessary.
  • Participate in crisis intervention services with individuals including mental status and risk assessment and linkage to crisis teams and hospitals. Assesses needs, develops and coordinates written crisis plans when indicated in coordination with client’s psychiatrist or APRN-RX
  • Maintains records of referrals, assessments and placement plans in compliance with HUD Housing Counseling Plan.
  • Enroll consumers into HMIS, complete VISPDAT assessment and attend CES meetings to advocate on behalf of consumers to expedite housing.
  • Provides on-going evaluation assistance, including Housing Assessments and ensure that documentation is upload into systems of record to include HMIS, SAMi and other systems as required by the program and the organization.
  • Provide supportive counseling or problem-solving activities to assist individuals in developing and maintaining the supports necessary to manage their illness
  • Develop and ensure that each client has a current individual service plan; develop a transition/discharge plan for each client no longer meeting the criteria for the level of care.
  • Respect and maintain the confidentiality of the guests/clients served.
  • Submit Monthly reports as requested.

Required Education and Experience
  • Bachelor’s Degree in Human Services, Social Work or related field from an accredited university.
  • One to two years of direct case management or supportive Housing services experience working with homeless or other disenfranchised individuals.
  • Valid Driver’s License and clean traffic abstract.

Salary : $39,000 - $49,300
