JOB TITLE: Children’s Health Home Care Manager
CLASSIFICATION: Full Time, Non-Exempt
PROGRAM/DEPARTMENT: Health Home Care Management
IMMEDIATE SUPERVISOR: Program Supervisor
POSITION SUMMARY:
Works with people with qualifying Physical/Mental Health/Substance Abuse/Developmental Disability diagnoses and their natural supports to link/monitor/refer to services that meet their individual needs and personal goals. Also provides ongoing advocacy. Provides ongoing advocacy through person centered approach. Meets documentation requirements of regulating organizations by completing documentation.
QUALIFICATIONS:Minimum Education, Training & Experience:
Associated Degree in Human Service related field
One to three (1-3 )years’ experience providing Care Management services (Case Management) to individuals with qualifying factors; i.e. physical/MH/ Substance diagnosis, two years’ experience developing Care Plans utilizing SMART goals.
Preferred Education, Training & Experience:The above plus:
Bachelor’s Degree in Human Service related field
Two (2) years’ experience developing Care Plans
Please Note: Applicants with less than a Bachelor’s Degree may be accepted but a waiver will be required for the organization.
ESSENTIAL SKILLS AND ABILITIES:
Valid New York State driver’s license and dependable transportation that meets the Agency standards
Ability to work flexible hours and travel throughout Chautauqua County
Ability to work effectively and independently including address multiple tasks efficiently
Strong organizational and analytical skills
Ability to think creatively and be results oriented
Ability to effectively communicate verbally and in writing including developing rapport with the people we serve
Willingness and ability to participate in an On-all rotation
Maintains assigned caseload, case size can vary.
Children’s Care Management: 18-23 average
SALARY:
$21 per hour starting rate
