Case Manager (QMHP)

Job Summary: Join our amazing team! Currently, we are looking for a Case Manager for our wrap facilitation position. The wrap facilitator enrolls new families into the program by performing a needs assessment and then meets with families weekly to provide case management. The wrap facilitator is responsible for conducting monthly team meetings to develop goals and strategies to help the family achieve their goals. This position is community based so personal transportation is required. Bilingual applicants strongly encouraged to apply!

This Case Manager position is within the Child and Family Services division of Integral Care in a program called Families with Voices. Families with Voices is a team-based program that provides wrap around facilitation for families experiencing challenges in housing, employment, mental health, child development and basic needs such as food and transportation. The team is comprised of wrap facilitators, housing stability specialist, a therapist and an employment specialist. We also coordinate with other agencies and supports that the family could benefit from. Families have access to any members of the team to help them achieve goals towards family stability.

Regular, predictable attendance is considered to be an essential requirement for this position. Other duties may be assigned. May require a flexible work schedule.

Education:

Required:

• Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field.

License:

Required:

• Current and valid driver's license and a clean driving record.

Experience:

• Bilingual candidates preferred. 

Hours: Full-Time.

Workdays: Monday - Friday.

Workhours: 8:00AM - 5:00PM (some evenings).

Salary: $22.35 - $23.47 an hour based on education.
