Facility Requirements:
- Must have BLS certification must be attached to submission packet (Must be through the American Heart Association)
- Must have 2 years RN Case Management experience, with experience in a long-term acute care setting. discharge planning, clinical case management & utilization review
- Must have Kentucky RN or multi-state RN license (must be attached to resume)
- Shifts are Monday - Friday 8a-5p; no on-call or weekends
