Position Summary:

     PRN RN Admission Nurse 

Works as part of the interdisciplinary team to provide a holistic approach to end of life care and symptom management. Creates a specialized plan of care, in collaboration with the patient, family and interdisciplinary group (IDG), to ensure quality care is provided and patient care needs are met.

Essential Duties and Responsibilities:

The following statements describe the principal activities of this position identified and shall not be construed as a detailed exposition of all the duties that may be inherent in this position.

Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family/caregiver to determine hospice needs. 

Provides a complete physical assessment and history of current and previous illness(es);

Provides professional nursing care,

Initiates the plan of care and makes necessary revisions as patient status and needs change;

Uses health assessments to determine nursing needs and any follow up needed;

Develops a plan of care that identifies interventions which will allow patient and family/caregivers to meet their goals;

Administers medications and treatments as prescribed by the physician in the physician’s plan of care;

Provides education to the patient and family/caregiver as appropriate per assessment and plan of care;

Assumes responsibility to coordinating patient care for assigned caseload;

Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given;

Records pain/symptom management changes/outcomes as appropriate;

Documentation will be completed and returned to the office within 24 hours of assessment;

Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required;

Assists the patient and family/caregiver and other team members in providing continuity of care;

Works in cooperation with the family/caregiver and IDG to meet the emotional and spiritual needs of the patient and family/caregiver;

Attends IDG meetings;

Ensures that equipment, supplies, medications and other necessary items and ancillary services are available and delivered/provided when needed;

Actively participates in quality assessment performance improvement teams and activities;

Participates in on-call duties as needed;

Be knowledgeable of admission process and paperwork;

May complete admission visits as needed;

Represent the Company in a courteous and professional manner at all times;

Complies with Compliance Plan;

Complies with HIPAA Policies. Maintains confidentiality of information relating to the patient and family. Will discuss only those aspects necessary to the care and treatment of patient and family with those directly involved in the patient’s care;

As business needs may change, may perform additional responsibilities assigned by supervisor that are reasonably related to the position.

Area of Responsibility:

Demonstrates good time management skills;

Performs initial patient assessment to include:

Documentation of initial assessment;

Effectively Communicates with interdisciplinary team members informing them of new admission;

Obtains initial physician orders;

Completes initial home/residence evaluation for safety, infection, emergency preparedness;

Conducts patient/caregiver education regarding home safety measures, infection control, and emergency preparedness;

Completes the initial plan of care;

Ensures symptoms are controlled, medications are ordered and refilled as needed, necessary equipment available or ordered, necessary supplies available or ordered;

Respects the uniqueness of living conditions and family dynamics of patients admitted to hospice services;

Contributes to the overall plan of care for patients by attending and participating in IDG meetings;

Ability to work flexible schedule to include some evenings, weekends and holidays

Education:

Graduate of an approved School of Professional Nursing;

Current state RN licensure in the state of practice.

Experience:

Hospice experience preferred but not required.

Knowledge, Skills and Abilities Requirements:

Must work well in a fast paced environment and adapt quickly to changing conditions;

Must have good problem-solving skills;

Must have basic math skills;

Excellent observation, verbal and written communication skills;

Knowledge of patient evaluation and triage procedures;

Demonstrate nursing skills per competency checklist;

Demonstrated ability to supervise aides;

Ability to provide care to patients at remote locations and demonstrate good nursing judgment with minimum supervision;

Ability to deal effectively and courteously with clients, families, and co-workers;

Ability to interact and exhibit an empathetic attitude toward the sick and elderly.

Physical Demands:

Frequent squatting/kneeling

Frequent bending/stooping

Adequate vision and manual dexterity

.Frequent standing/walking

Frequent lifting/carrying up to 25 pounds

Frequent pushing/pulling up to 50 pounds

Occasional lifting/carrying up to 50 pounds

Occasional pushing/pulling in excess of 100 pounds

Occasional reaching above shoulder level

Driving automotive equipment for business necessity

Work Environment:

May involve exposure to potentially dangerous materials, infectious diseases, and situations that require following extensive safety precautions and may include the use of personal protective equipment. Employees may be required to adapt to conditions including, but not limited to, temperatures in excess of 80 degrees and below 55 degrees, humidity, working around client’s families or pets, cigarette smoke or dust. May involve fast paced and occasionally stressful work environment with tight deadlines;

Complies with the agency dress code, projects a professional attitude toward self, patients, and other staff members, and follows department public relation policies regarding outside speaking and professional affiliations, clearing all such requests through the agency.

Conditions of Employment:

Successful candidate must have a satisfactory post-offer Medical Review;

Must pass a pre-employment criminal background check; 

Must have proof of Auto Insurance (minimum required by law), valid driver’s license, and reliable transportation;

May be required to complete a biennial health assessment or physical exam and submit to annual TB screenings and/or Hepatitis vaccinations, as well as possess a current CPR certification through an approved credential source;
