What are the responsibilities and job description for the RN Case Manager position at Interim HealthCare - New Hampshire?
Interim HealthCare is continuing to grow and we are looking to add compassionate nurses to our team! We are seeking several full-time RN Case Manager's based out of our Manchester, NH office. The RN Case Manager position is responsible for managing a defined caseload of patients providing direct patient care and comprehensive nursing services to individuals in the community.

OUR CURRENT NEED INCLUDES: (COVERAGE AREA WILL BE ONE OF THE FOLLOWING)
- Territory to provide coverage across the greater northern Manchester, NH area
- Territory to provide coverage across the greater southern Manchester, NH area.
- Territory to provide coverage across the Derry, Londonderry and Salem, NH area
- Territory to provide coverage across the Merrimack and greater Milford, NH area
- Territory to provide coverage across the greater Nashua, NH area
Job Requirements
- Valid nursing license in the state of NH
- 2 years of experience in skilled care preferred
- 1 year of experience in nursing supervision preferred
- Recent wound care experience
- Current CPR certification
- Reliable transportation
- Home care experience preferred
