Job Summary
The Community Liaison/Patient Care Coordinator is responsible for generating new patient referrals, attaining monthly goals, and increasing patient census. The Liaison will interact and develop relationships with hospitals, skilled nursing facilities, physician offices, and assisted & independent living facilities. Developing relationships with discharge planners, social workers, case managers, and patients and their families is the key to generating patient referrals, along with providing community education and consistent follow-ups.
Sales/business development experience in home health, hospice or the healthcare industry is highly valued but not required.
Responsibilities and Duties

- Communicates clearly and effectively the vision and services of the company
- Markets confidently, consistently and deals tactfully with clients and community partners
- engages in and is aware of community events that would prove effective in educating prospective clients of company vision and services
- Follows up with referral sources, and keeps them up to date regarding patient status and condition post-discharge from their facilities
- Ensures patient retention in home health by educating patient, their family and caregiver as to the benefits of home health service
Qualifications and Skills
- Excellent verbal and written communication skills
- Self-motivated, able to work independently and without prompting
- Confident and self-assured
- Valid state driver's license
- Reliable automobile
