Job description
Please note that this is a per diem position that is paid on a per visit basis. It is ideal for an OT looking to supplement their income, pick up additional hours and does not want a set shift type of schedule. You will be expected to visit patients in their homes with in Santa Clara, Alameda or San Mateo counties.
Essential Job Functions
- Evaluate patient's functional status including muscle function, endurance, visual coordination, written and verbal communication skills, self-care ability, work capacity etc.
- Evaluate patient's home for hazards and barriers to independent living , identifying any equipment needs or home modification needs
- Develop a treatment plan and establish goals for improved function - communicates this with physician and other care team members to ensure not overlap of care
- May teach new skills or retrain patient in familiar ADLs that have been lost due to illness or injury
- May train patient in use of orthotic or prosthetic deices
- Maintains appropriate clinical records, notes, and reports changes in condition to physician and agency, adhering to agency policy
- Maintains care coordination and communication with agency staff with regard to patient's needs and progress
- Attends and contributes to in services, case conferences and additional meetings as required by agency policy
- Supervises COTAs according to agency policy
- Participates in discharge planning for patient
Qualifications
- Licensed by the California Board of Occupational Therapy
- Registered with the National Registration Examination of the American Occupational Therapy Association
- 2 Years of experience as an OT
- Home health experience preferred
- Demonstrates good verbal and written communication and organization skills
- Possesses and maintains a current CPR certification
- Must be a licensed driver w/an automobile that is insured in accordance with state requirements and is in good working order
Salary : $100 - $130
