Issac. H. Tuttle Fund, a private foundation seeks LMSW to join our stipendiary program which provides financial assistance for formerly self supporting Manhattan residents 65 who need ongoing financial support to continue living safely and independently in their own homes.
Key Responsibilities
Oversee and maintain individualized care plans,benefits and budgets.
Coordinate social, medical, legal and home care services as needed.
Conduct reassessments and budget reviews
Assist with housing transitions and end of life issues.
Maintain accurate documentation and participate in case reviews.
What we are looking for
Licensed Master Social Worker
Experience in aging services and case management.
Strong collaborative critical thinking skills
Proficiency with productivity software , commitment to ongoing skil development.
Salary and Benefits
$70,000 -$75,000 depending on experience
