What are the responsibilities and job description for the Care Transitions Specialist II position at JASA?
Shift Schedule: Monday-Friday 9AM-5PM
Hours Per Week: 35
The Program Director will be responsible for providing supervision to the Care Transitions Care Transitions Specialists, overseeing day to day management of programs, and serving as a key contact for healthcare payer and provider partners including Healthfirst, Maimonides, SUNY Downstate and Emblem.
Care Transitions DUTIES AND RESPONSIBILITIES : In accordance with the standards of the transitional care program and requirements of the funding partners, responsible for:
Provide supervisory oversight of the care transitions services rendered by the Health Services program;
Supervision of care transitions specialists and the direct service to clients related to the development and implementation of a written case plan for the provision of the full range of the care transitions services
Assist in the recruitment process and train new staff
Identify new program opportunities and challenges and discuss potential responses/solutions with Associate Director and Senior Director
Maintain oversight and monitor the status of the cases identified as elevated risk. That includes developing a plan with case managers ensuring that services are in place for these clients.
Maintain oversight and monitoring of any clinical event notifications, including updating the care plan.
Monitoring of staff and case review with regard to ongoing assessment of client need, monitoring of service efficacy, appropriate revision of the care plan, and case compliance.
Informing Associate Director of emerging issues and/or crisis situations with clients; work with care transitions staff to develop a plan to resolve the risk.
Maintain a caseload and direct provision of care transitions services.
Represent the agency at community conferences and meetings.
Participate in training and unit meetings.
Complete required case records, reports and statistics.
Serve as a key contact for healthcare partners (e.g., Healthfirst, Emblem, Maimonides)
Perform such other duties as assigned.
Qualifications
A licensed master’s level graduate healthcare professional or international medical graduate (M.D. credential) with prior experience in a behavioral health clinical setting or care management supervisory capacity; OR a master’s level professional with three years prior experience supervising clinicians and/or care managers providing direct services to people with SMI or serious substance use disorders
Demonstrated organizational skills and ability to provide professional supervision and leadership to social work staff.
Ability to read, write, speak, understand English. Good writing/record keeping skills.
Computer literate, with knowledge of current Microsoft Office Suite and Google applications, plus significant experience performing Internet-based research
Ability to travel to clients' homes using mass transit and to negotiate any steps and barriers in clients' residences.
Understanding and appreciation of the roles of race, religion, ethnicity, sexual preference, and individual values as they relate to serving clients and families.
