What are the responsibilities and job description for the Registered Nurse Case Manager position at JCCA?
Job Summary:
The Registered Nurse Case Manager monitors each child’s health, medical treatment, and follows up with the medical provider and foster and biological family; provides support, counseling and training to foster parents in health and medical needs of children; coordinates internal services with outside medical agencies; makes monthly home visits as well as specialty clinic appointments; maintains and updates all required files and records.
Essential Job Functions:
- Communicate and coordinate with foster and biological families/guardian about all medical conditions, appointments and/or treatments.
- Participate in community visits (e.g. Emergency Room, hospitalizations, community specialty appointments, home visits) on as needed basis.
- Ensure full compliance in program and records with internal and external and regulatory requirements; obtains health records, Newborn Screening record, birth record, immunization registry and other pertinent medical or mental health information; reviews ACS triage material and communicate with ACS on as needed basis.
- Participate in all staff meetings, case reviews, FTC, etc.;
- Provide formal and informal training for foster and biological parents to instruct them in appropriate health practices and administration of medication.
- Provide training workshops and seminars on special medical issues for staff.
Knowledge, Education and Skills Required:
- Associates Degree required; Bachelor Degree in Nursing or related field is preferred
- NYS Registered Nurse License
- 1-2 years ICU experience (pediatrics preferred) years or acute and sub-acute care; visiting nurse services preferred.
- Bilingual (Spanish) is HIGHLY preferred
