SUPERVISOR: Chief Medical Officer
HOURS: M-TH 10 hrs per day/4 days per week (flexible) between 7am-7pm
CLASSIFICATION: Full Time/ Hourly/Non-Exempt PRIMARY LOCATION: 1021 Broadway
QUALIFICATIONS:
● Must have Doctor of Physical Therapy degree and be licensed in NYS
● Must have current CPR/AED and First Aid certification or will obtain through Jericho Road Community Health Center
JOB SUMMARY: Our Physical Therapists are a part of our Medically Oriented Gym team, which strives to make movement and daily living accessible to everybody and everybody. Physical Therapists will treat patients from a diverse background and be expected to work with multiple patients at once.
Essential Job Duties
PATIENT CARE
Diagnose and treat patients of all ages and backgrounds to help them optimize performance of daily living activities.
Develop treatment plans to aid in reducing pain, restoring function, increasing mobility, and preventing future morbidity and be able to modify treatments when appropriate in order to achieve these goals.
Evaluates and records patient’s progress, modifying treatment plans, and trying new treatments if required.
Instructs patients and teaches them therapeutic exercises, stretches and a range of motions, flexibility, coordination, and balance to improve their condition.
Conduct appointments to identify potential and existing problems as well as determining whether the problems at hand are appropriate for physical therapy.
Participate in preparing physical therapy patients to transition to the gym once their physical therapy treatment is complete.
The therapist will only perform interventions within their scope of practice in which they are competent and/or received advanced continuing education to perform.
Provides ongoing education and instruction through a written home program to the patient and caregiver.
Maintain patient confidentiality
COLLABORATION & ADMINISTRATION
Communicate with an interdisciplinary team (including primary care providers, community health workers, care managers, administrative staff, etc.) to ensure consistent care to address all aspects of health.
Participate in regular metric reporting as a team and comply with federal and state physical therapy regulations.
Provides and supervises growth and learning opportunities for and/or physical therapy students and/or interns
Work with credentialing team on all aspects of information as needed
Aid in the opening and closing of the facility
Must remain current in best practices in industry and in licensure
Completes required continuing education requirements as mandated by state licensing.
Chart and track client progress in the EHR (Medent) system
