Responsibilities
- Responds to initial crisis calls and identifies needs & necessary resources accordingly.
- Uses established best practices to engage formal and informal resources for the client, including the transition, discharge, and aftercare plans of behavioral-health services and demonstrates a positive, strengths-based attitude of teamwork and shared leadership.
- Performs ongoing case management and coordination of services to children and adults within the Child & Family Team process in accordance with the Plan.
- Assists clients and families in obtaining community support, identifying resources, and resolving barriers.
- Provides coordination of behavioral-health services with other agencies, such as Department of Child Safety, Probation/Parole Departments, Division of Developmental Disabilities, Arizona Long Term Care, schools, and other providers.
- Bachelor's Degree in behavioral health field preferred; a combination of experience and education may be considered
- Relevant experience working in a behavioral-health setting
- Level 1 Fingerprint Clearance Card
- Verification of education, employment, references
- MVR matching JFCS driving guidelines
- AZ Driver's License and Valid Auto Insurance
