Overview
The Direct Care Worker plays a crucial role in providing compassionate and personalized care to individuals with varying needs, including those with disabilities or requiring assistance in daily living activities. This position is essential for enhancing the quality of life for clients in group homes or community settings. The ideal candidate will have a strong commitment to supporting individuals in achieving their personal goals and maintaining their independence.

A Direct Service Worker must be able to implement individualized plans that reflect person-centered choices and self-determination concepts for participants in the Adult Day Health (ADH) community-based program and workshop program. The DSW reports to the Service Supervisor. Specific responsibilities are as follows:

Duties

  • Assure client's rights are protected.
  • Protect participants from abuse and neglect, which must be reported to the Service Supervisor of any conditions of abuse upon the start and end work shift.
  • Notifies and documents any concerns or problems regarding the participant's physical, mental, social status, and interests.
  • Must be able to read, prepare reports, and record daily pertinent information on participant's progress and attendance. As necessary, be able to thoroughly complete appropriate reports: Incident & Adverse Event report, Interdisciplinary Entry, Case Notes, and Date Flow Sheets.
  • Must comply with policies and procedures outlined in the Employee Handbook (sick leave policy, swapping shifts, dress codes, submission of case notes and flow sheets, etc.) and in the DHS Medicaid Waiver Contract.
  • Be familiarized with assigned participant's medical history and Individualized Plan (IP). Must be able to implement IP accordingly and adhere to confidentiality of participant's information.
  • Be familiarized with assigned participant's caregiver, case manager, guardian, or other members of participant's Circle of Friends (CF).
  • As directed, attends and actively participates in staff meetings, IP conferences, and scheduled staff training sessions.
  • Provides team support in implementing center-based and community-based activities.
  • Must possess knowledge and skills necessary to provide appropriate care to clients with intellectual and developmental disabilities.
  • Implement and follow care plans tailored to each participant's specific needs.
  • Must be able to calmly resolve problems, handle conflicts and make effective decisions under pressure. Must be mentally adaptable and flexible in dealing with a variety of people.
  • Must ensure the health and safety of participants and staff within their surroundings.
  • Assist feeding, teeth brushing, dressing, grooming, changing diapers, and bathing of incontinent participants, assigned to the employee.
  • With assistance, may require lifting and carrying participants from sitting to standing position, and assisting in their movements or other ambulatory duties.
  • Must be ready to administer First Aid or Cardiopulmonary Resuscitation (CPR), and be able to secure and ambulance as needed.
  • Must be able to sit, stand, stoop, bend, push, or lift participant and equipment. Requires the manipulation of objects, such as grasping with hands/wrists, reaching for objects above should height or other activities depending on the environment where training is taking place.
  • Must be able to communicate verbally and in writing with members of the team and other disciplines. Must have compassion and tolerance to work with participants, staff, administrations, and others to ensure that participants' best interests are first taken into considerations.
  • Must ensure sufficient number of employees are supervising participants, in accordance with the maximum staffing ratio provided by State of Hawaii, Department of Health Developmental Disabilities Division.
  • Effectively communicate with other employees regarding supervision of their designated participant(s) during breaks, ISP conferences, Community-Learning Service-Group (CLS-group), or other activities that may leave participant9s) unattended. Supervision of participant(s) must always be ensured at all times, either from DSW employee or administrative staff.
  • Provide companionship and emotional support to clients, fostering a positive and encouraging environment.

Requirements

  • Experience in long-term care or home community care is preferred.
  • Familiarity with working with individuals with disabilities is highly desirable.
  • Strong communication skills to effectively interact with clients, families, and other healthcare professionals.
  • Compassionate demeanor with a genuine desire to help others.
  • Ability to work collaboratively within a team environment while also being self-motivated.
  • Flexibility in scheduling to meet the needs of clients.

Job Types: Full-time, Part-time

Pay: From $16.00 per hour

Expected hours: 30 – 40 per week
