Field RN Case Manager for Skilled Home Care (OASIS) for either Bronx or Manhattan.
What you will be doing :
- Field RN Case Manager providing home health visits for skilled nursing care
- Admissions, OASIS, create plan of care, care coordination, re-certifications, discharges
- Provide supervision, skilled nursing care
- Ensure documentation is completed
- Provide clinical care to patients as needed
- Home Health experience highly preferred or hospital experience
- NY state RN license
Skills you need :
Salary : $105,000 - $130,000
