TARGETED CASE MANAGER/CBRS

Case Management and Community Based Rehabilitation is a community-based program that involves a behavioral health professional, to assist clients with Mental Illness, who are at risk of being underserved in their effort to identify, access and utilize medical, behavioral health or social services, or to otherwise achieve goals to attain recovery/resiliency. CBRS will work toward providing education towards skill building activities for individuals with Mental Illnesses to meet their goals and achieve a higher level of independence and quality of life.

Targeted Case Manager and CBRS Responsibilities:

  • Utilize the comprehensive diagnostic assessment of a person’s strengths, deficits and needs using information gathered from the client, family, and other past and present service providers.
  • Develop and implement a written service plan to address the needs of the recipient.
  • Accurately document the billable services they perform in the case record. The record must be kept current, complete, and accurate at all times.
  • Perform 90 day and annual reviews within the mandatory time frames.
  • Provide services in an ethical manner as outlined in the NASW Code of Ethics and Medicaid.
  • Respond to clients’ needs and requests for assistance in a timely manner (within the day).
  • Make all scheduled appointments with clients, and/or notify clients in emergent situations.
  • Work with other professionals in the community in a professional capacity to access needed community resources.
  • Understand and practice within the Optum Guidelines, NASW guidelines and Health and Welfare guidelines pertaining to Targeted Case Management Services and Community Based Rehabilitation.
  • Following the initial 30 day training period, workers are expected to fall into the 24-hour on-call phone rotation. On-call workers respond to emergency calls after hours for a one week period.
  • Complete supervision with a licensed clinical professional who is employed by JDSA, at a minimum, of once a week.
  • Complete a minimum of 20 hours of continuing education in the mental health field per year. Training will meet the guidelines set forth by the Idaho Bureau of Occupational Licenses and by Optum Idaho.

Qualifications Targeted Case Managers and CBRS must complete a minimum of a Bachelors degree in a Human Services field from an accredited institution.

It is understood that the Individual Case Manager and CBRS will consult with their supervisor about any issue they do not fully understand.

Case Managers are compensated on the billable hour system, and most activities required for the position are billable activities. However, it is mandatory for Case Managers to participate in weekly supervision and the Quality Assurance Program in order to assure that the above responsibilities are being met. While these are not billable activities, they are designed to assist workers in keeping up to date and accountable within their practice. Supervision and the QA program are designed to require minimal time investment on the worker’s part.

Job Type: Full-time

Pay: $17.50 - $21.00 per hour

Expected hours: 20 – 35 per week
