SUMMARY/ OBJECTIVE

The MDS Coordinator participates in the Resident Assessment Process through completion of the pertinent items on the Minimum Data Set (MDS) and the accompanying Care Area Assessments (CAAs). Participates in developing individualized resident Care Plans, identifying the needs of the resident and projected outcomes as required by Federal and State regulations.

ESSENTIAL FUNCTIONS

1. Conduct and coordinate the MDS and Care Plan as outlined by the facilities’ policies and procedures.

2. Responsible for informing all care plan team members of when a care plan is due and ensuring that all care team members complete their portion of the MDS.

3. Develop and/or revise resident care plan quarterly and with any significant change in condition.

4. Responsible for all PPS and state Minimum Data Set required assessments. Ensure that all residents have the appropriate MDS, CATs, and CAAS completed.

5. Coordinate all care plan meetings and encourage team members to participate in the care planning process according to policy.

6. Responsible for accurate and timely completion and submission of MDS to state/federal agencies according to Medicare/Medicaid guidelines. Responsible to initiate and maintain the

7. Resident Assessment and Care Plan Schedule ensuring all dates for MDS and CAAS completion are met in accordance with state/federal guidelines.

8. Ensures residents’ rights are observed in the MDS Process, such as confidentiality and privacy.

9. Maintain effective communication among departments so MDS and care plans can be kept current when changes to the residents’ condition occur.

10. Monitors resident care during the MDS process and reports concerns to the Facility Administrator and DON.

11. Assists with the pre-admission screening process to estimate the potential resident’s RUG’s group, as needed.

12. Reports contraindications between observed resident status and documentation in the residents’ medical record to the Director of Nursing.

13. Other duties as assigned.

EDUCATION/VOCATIONAL REQUIREMENTS

1. Must hold a current nursing license in the state of employment.

2. Experience in Long Term Care preferred, and previous MDS Coordinator experience preferred.

3. Computer skills such as Data Entry and Word processing required. Working knowledge of the MDS 3.0 (current version of minimum data set).

4. Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations and guidelines that pertain to skilled nursing facilities.

5. Must possess the ability to plan, organize, develop, implement and interpret the programs, goals, objectives, policies and procedures that are necessary for providing quality care.

JOB SKILLS:

1. Must be in good physical and mental health.

2. Must have knowledge of procedures and techniques involved in administering simple treatments and providing related bedside resident care services, as permitted by state regulation.

3. Must pay attention to safety hazards to residents and self.

4. Must be familiar with location of various departments and regulations of the Center.

5. Must understand standard techniques used in providing personal services for residents and in caring for equipment and supplies.

6. Must have a good understanding of basic medical asepsis, sterile techniques and standard precautions to avoid infection of residents and contamination of equipment and supplies.

7. May be required to work rotating shifts and to work in different units as residents needs require; specific scheduling will be under the direction of the nursing supervisor.

8. Must possess the ability to positively interact with personnel, residents, family members, visitors, government agencies/personnel, and the general public.

9. Must provide annual verification of a negative tuberculosis (TB) skin test.

COMPLIANCE

1. Complies with applicable legal requirements, standards, policies and procedures including but not limited to those within the Compliance and Ethics Program, Standard/Code of Conduct, Federal False Claims Act and HIPAA.

2. Participates in required orientation and training programs.

3. Promptly reports concerns and suspected incidences of non-compliance to supervisor, Compliance Liaison or to the Compliance Officer.

4. Cooperates with monitoring and audit functions and investigations.

5. Participates, as requested, in quality assurance and process improvement activities.

OTHER JOB DEMANDS

Physical Environment:

Must have the ability to sit for long periods of time while at the computer station entering information. Workspace is inside the facility.

Working Conditions:

Ability to organize and prepare records and charts according to policy. A preference for scientific and technical activities in understanding and responding to medical problems and concepts. Ability to interpret operating policies/procedures and to review work performance in determining compliance to these recognized standards. The potential for exposure to bio-hazardous waste and blood borne pathogens exists. Must have ability to assess residents, document findings and have good clinical observation skills.

Physical Effort:

Lifting up to 50 lbs. Maximum with frequent lifting of objects weighing up to 25 lbs. Frequent reaching, handling of instruments, equipment, records and reports. Long periods of time at the computer. While standing, stooping, and bending occurs when conducting assessments of residents.
