Qualifications :
- Graduate of an accredited seminary or school of theology and / or a certificate of ordination or commission from a religious faith group; completion of Clinical Pastoral Education (CPE) preferred
- Previous hospice experience working with death and dying strongly preferred
- Comfortable in an interfaith setting and able to accept different lifestyles, cultures, beliefs, and values
- Maintains a current CPR certification
- Valid drivers license and proof of insurance is required
HOURS : Monday-Friday 8 : 30am-5pm
On-Call Expectations : 2-3 time / month
TERRITORY : Gurnee, Libertyville, Mundelein, Lake Villa, Johnsburg
Salary : $57,000-$70,000 Annually - Offer Based on Years of Experience

Performs spiritual bereavement assessments of patients and families / caregivers in the hospice program in a timely manner and ensure that identified needs are addressed in the patients plan of careProvides direct spiritual care and bereavement counseling to patients and families / caregiversDemonstrates exceptional service to patients, families, referral sources, co-workers, and other customers. Assists team members as needed to ensure that this level of service is consistently providedProvides accurate and timely documentation of patient services to reflect the plan of careAttends group meetings to facilitate coordination of care and discussion of interdisciplinary group involvementProvides consultation, education, and support to the interdisciplinary group on spiritual care and bereavement counselingParticipates in Continuous Care and on call, if necessaryAssists in supervision of spiritual care volunteers when assigned to patients families / caregivers as neededAssumes responsibility in directing the development and implementation of the bereavement plan of care and the frequency of service delivery; make available bereavement services for up to one year following the death of the patientText 7639 to 847-416-8078 to APPLY!
