What are the responsibilities and job description for the Medical Records Clerk position at JWCH Institute?
POSITION PURPOSE:
Under the supervision of the Program Manager, the medical records clerk will perform specialized duties such as pulling charts, trouble shooting all missing charts, data entry, and assembling and maintaining medical records in accord with established guidelines, policies and procedures.
PRINCIPAL RESPONSIBILITIES:
- Responsible for maintaining medical records agency-wide in a standardized and professional manner in order to protect patient confidentiality and to promote quality patient care.
- Ensure that information contained in the record is complete, accurate and only available to authorized personnel.
- Ensure that records are released in accordance to state and federal laws.
- Work and collaborate with Health Plans to ensure audited charts for HEDIS and other audits are submitted and reviewed in accordance to state laws.
- Maintain the Rosetta Tank for JWCH and ensure lab results are matched with lab orders in Next-Gen (EHR system).
- File and manage medical charts in terminal digit order.
- Attach correspondence to the chart under appropriate tab categories. This requires a comprehensive knowledge of filing all types of medical reports in the patient’s chart in EHR.
- Conducts quality control on all active medical charts.
- Trouble shoots all missing charts until they are located or a disposition can be obtained.
- Search for medical records and file medical documents into each record. This requires a comprehensive knowledge of filing all types of medical reports into the patient’s chart in EHR.
- Answer routine inquiries by telephone or in person concerning medical records information.
- Answer calls in a courteous, efficient and professional manner.
- Obtain and verify patient demographic information.
- Adhere to strict standards regarding client confidentiality, informed consent and disclosure as required by law and agency policy.
- Assemble and mail information to patients.
- Verify eligibility via the state Medi-Cal, Health Plan and MHLA websites as needed.
- Perform other reasonably related duties as assigned by supervisor.
REQUIREMENTS:
- Experience in community health care setting preferred.
- Excellent customer service skills.
- Ability to multi-task in a fast pace environment and work in a team environment to meet goals.
- Very strong verbal communication skills with pleasant voice manners.
- Ability to relate and communicate well with a diverse patient population.
- Types at least 40 wpm.
- High School diploma or GED.
- Bilingual (Spanish/English).
PREFERRED:
- Graduate of an approved medical assistant program in the state of California
- CPR Certificate. (Training may be provided.)
PHYSICAL DEMANDS AND WORKING CONDITIONS:
- Extensive sitting.
- Extensive typing on the keyboard.
- Constant interactions on the phone.
- Occasional lifting and carrying items weighing up to 35 pounds.
- Requires manual dexterity and eye-hand coordination.
