Hourly Pay: $43.00 - $46.96
Work Schedule:
- Monday - Friday
- 8:00 a.m. - 4:30
- May work weekends or other shifts periodically as needed.
Duties:
The MDS (Minimum Data Set)/Care Plan Coordinator is a licensed, registered nurse who manages patient assessments and care plans in the long-term care facility. This RN position is responsible for ensuring that KVH is compliant with federal and state regulations, and that KVH is positioned to receive the maximum reimbursement from Medicare and Medicaid for the level of resident care provided by nursing staff. The MDS Coordinator will conduct assessments, create and modify care plans, determine care levels and code accordingly in the MDS and will provide supervision over the Restorative Nursing Program. The level of resident care may range from minimal assistance to total care. Other general responsibilities will include coordinating aspects of resident care as a member of the interdisciplinary team, training staff, and communicating resident needs with other team members.

Requirements:
- Licensed to practice as a Registered Nurse (RN) in the State of Kansas
- Have one (1) year of experience in registered nursing.
- Must pass a criminal background screening and be free from conviction of a criminal offense that would prevent you from employment with KVH.

- Three (3) years of positive work history in the nursing field with at least six (6) months of experience working with a geriatric population.
- One (1) year supervising subordinate staff members.
- Excellent written and oral communication skills.
- A helpful and sympathetic attitude toward patients.
