Responsibilities & Requirements
The Kettering Health Clinical Pharmacist – Specialty Pharmacy works alongside the multi-disciplinary oncology team to educate, monitor, and support patients that are receiving oral oncolytic agents. This clinical pharmacist position is responsible for the Specialty Pharmacy Patient Management Program as well as serving as a clinical resource in a multi-disciplinary setting in the Kettering Cancer Care service line. The clinical pharmacist ensures patients receiving oral chemotherapy are provided education, assistance with access to the medication, appropriate follow up, and successful maintenance of therapy based up on individualized treatment goals. The clinical pharmacist works with the Specialty Pharmacy Manager to assess current systems for appropriateness and efficacy as well as develop and implement new systems within the Specialty Pharmacy Team to improve patient outcomes, increase patient and provider satisfaction, and meet or maintain any legal or regulatory requirements for the pharmacy. Performs other duties as assigned.

Education
- Minimum Education: B.S. Pharmacy and/or Pharm.D. from an ACPE accredited College of Pharmacy
- BLS
- Possesses a valid Ohio pharmacist license, or be eligible to receive licensure
- Ability to develop strong interpersonal relationships
- Flexible attitude and ability to adapt to schedule and workload changes
- Professional presentation and demeanor
- Collaboration skills
- Collaborate with hematologists/oncologists, advanced practice providers, and other clinical staff to provide patients with individualized treatments utilizing the most recent and relevant evidence
- Oversight of the Specialty Pharmacy Patient Management Program
- Ensures pharmacy is meeting standards required by accrediting bodies and legal entities
- Membership and participation in Specialty Pharmacy Quality Management Committee (QMC)
- Aggregating and reviewing performance / prescription metrics
- Developing and implementing quality improvement plans as deemed necessary by QMC
- Pharmaceutical management of all clinic patients taking oral oncology/hematology agents to ensure appropriate medication use
- Identifies, resolves, and prevents medication-related problems, including subtherapeutic dosage, overdose, drug-drug interactions, drug-disease interactions, adverse reactions, inaccuracies, and inappropriateness.
- Provides education at onset of therapy and appropriate follow up based on goals of therapy / efficacy / tolerability
- Provides communication between clinic and Specialty Pharmacy
- Document appropriately in patient medical records
- Assist with medication access as part of Specialty Pharmacy Medication Assistance team
- Completion of a post-graduate year 1 residency (PGY1), fellowship or 2 years of relevant post-graduate pharmacy experience required in oncology – preferred
- Previous clinical pharmacy experience is preferred.
- 3 years of experience in oncology preferred.
