The After Care Counselor is responsible for the coordination of continuing care arrangements for patients of the program. Primary responsibilities include: The provision of a safe and therapeutic milieu; coordination of aftercare arrangements and housing placement in clinically appropriate locations. The delivery of individual and group educational services, crisis intervention and full participation with the multidisciplinary team toward meeting a patients treatment plan objectives.

Qualifications
Job Requirements:
- Prefer a Bachelor or Associates degree in a related mental health discipline from an accredited college
- required High School Diploma or equivalent with experience in a chemical dependency and or mental health treatment setting with experience in aftercare.
