What are the responsibilities and job description for the Plebotomist position at Keystone Health?
Keystone Health, located in Eagle, is currently searching for a Certified Medical Assistant and/or Phlebotomist to join our Chronic Care Management team in the field.
The ideal candidate is reliable, detail-oriented, and self-motivated. This position involves traveling to perform specimen collection along with additional patient care services including taking vitals, medication placement, and other tasks ordered by the provider. The position would also entail monitoring patient health status, following up on directed care by providers, and orchestrating seamless care for our patients. The CCM program collaborates directly with providers in a variety of healthcare settings to identify patients with chronic conditions and alleviate gaps in care.
Qualifications:
- Medical Assistant Certificate and/or Phlebotomy Certification
- Minimum of 3 years (5 years preferred) of experience preferably with chronically ill patient population.
- Proficient use of Microsoft Word, Microsoft Excel and EMR systems (ECW experience a plus)
Skills and Abilities:
- Knowledge and understanding of HIPAA
- Ability to perform venipuncture and capillary collection primarily on the geriatric population
- Collect and prepare specimen samples for the laboratory
- Demonstrate strength in documenting within an EMR
- Ability to effectively problem-solve
- Demonstrate excellent verbal and written communication skills
- Strong organizational skills
