Overview
We are seeking a dedicated and skilled Wound Care Nurse (RN or LPN) to join our healthcare team. The ideal candidate will have a strong background in patient care, particularly in wound management, and will be responsible for assessing, diagnosing, and treating patients with various types of wounds at home. This role requires a compassionate approach to patient care and the ability to work collaboratively with other healthcare professionals to ensure optimal patient outcomes. Responsibilities
- Conduct thorough assessments of patients' wounds and overall health status.
- Develop individualized care plans based on diagnostic evaluations and patient needs.
- Administer appropriate wound care treatments and dressings.
- Document all patient interactions and maintain accurate medical records.
- Collaborate with interdisciplinary teams including physicians, nurses, and specialists to coordinate comprehensive care.
- Educate patients and their families on wound care techniques and prevention strategies.
- Monitor patients for signs of infection or complications related to their wounds.
- Provide support in emergency situations, utilizing skills from previous experience in ER or ICU settings when necessary.
Requirements
- Valid nursing license (RN or LPN) in the state of practice.
- Experience in wound care management is preferred, espicalliy Wound Vac applications; certification in wound care is a plus.
- Familiarity with medical records systems and documentation practices.
- Strong diagnostic evaluation skills to assess wound conditions effectively.
- Background in hospital settings, particularly in areas such as ICU or cardiac catheterization, is advantageous.
- Experience administering injections as part of wound management protocols.
- Knowledge of behavioral health considerations related to chronic wound patients is beneficial.
- Excellent communication skills and the ability to work well within a team environment.
