Job Responsibilities:
Act as a single point of contact for all parties involved in their client's well-being and care, with the goal being to facilitate care coordination throughout the course of their involvement in the residential treatment facility.
Arrange medical appointments and transportation for clients as required
Discuss a clients’ treatment plan with other professionals and perform administrative duties related to a client's care and arranging other services as needed throughout the duration of the client's residence in the facility.
May help clients accomplish this goal by assisting them with practical needs like finding housing, helping them find employment, facilitating short-term disability, and arranging for continuing outpatient treatment after being discharged from the residence, if needed.
· Preferred two-year’s experience in a behavioral health position.
Schedule: Monday - Friday 9:00am - 5:00pm
Job Type: Full-time
Pay: $37,500.00 per year
