Important Details: Bonuses are paid out in 2 payments: $1,750 at 30 days after hire and $1,750 at 6 months after hire. To be eligible, you must be a non-BILH employee or a previous eligible employee who returns to BILH after 1 year. Employee must be in good standing to receive the bonus at the time of payment. All bonuses are subject to applicable taxes. If you leave Lahey Health & Medical Center before your first anniversary you will pay back the sign-on bonus to Lahey Health & Medical Center. Job Description:
Lahey Hospital & Medical Center is seeking a Lic. Clinical Social Worker to support the Transplant Department. We are seeking clinician to support the ambulatory clinic. Successful candidate will conduct initial evaluation and subsequent regular follow-up assessments of transplant recipients functioning, compliance and psychosocial needs. Provide interventions as identified in assessment to address any psychosocial problems/barriers that interfere with post-transplantation plan of care. Provide counseling, psycho-education, groups, support, crisis intervention and other psychosocial services to patients and their families in order to help them cope with psychosocial stresses related to end stage Liver disease and adjustment to Transplantation. Assist patients through advocacy procedures and referrals to community agencies as indicated. Experience working with end stage liver or kidney disease or transplantation population a plus.
