We provide services to 24 surrounding towns, and nearly 4,000 patients each year. We provide integrated mental and physical health care to children, adults, elders and their families who are living with – and recovering from- mental illness and/or emotional distress. - Provide supported employment and vocational case management services to Assertive Community Treatment (ACT) Team patients as part of the collaborative and integrated ACT Team.
- Provide direct and indirect services that actively facilitate competitive job acquisition and retention based on patient preference and abilities.
- Employ evidence based, supported employment practices in the delivery of services.
- Provide on-site job support and supervision as indicated.
- Conduct job development activities.
- Provide Functional Support Services as appropriate.
- Assist patients in maintaining community living by teaching them activities of daily living and personal care.
- Promote independent and productive habits of patients in the community.
