JOB DESCRIPTION

 

Title:  Substance Use Disorder (SUD) Community Case Manager (CCM)                         

Department: Community Support Program (CSP) and SUD                                           

Reports To: CSP Case Management Facilitator

 

Primary Purpose:

To provide support with a recovery focus to adult patients experiencing co-occurring mental health and substance use disorders and to assist patients to discover and maintain a valued role(s) in the community. 

 

DESCRIPTION OF DUTIES & TASKS:

Under the general direction of the CSP and SUD Program Directors, and the general supervision of CSP CM Facilitator, and within the center’s program policies and procedures, performs the following functions.

 

Essential Functions

  1. Assists patients to maintain community living by teaching activities of daily living and personal care; promotes independent and productive habits to patients in the community. 
  2.    Completes case management assessment and develops comprehensive care plans and individual treatment plans and monitors services received.  Performs targeted case management services as defined by the HeM’s in assisting individuals in gaining access to needed medical, social, educational and other services.
  3.   In collaboration with Prescriber, Emergency Services, families and natural supports, provides ongoing assessment, monitoring, support and crisis interventions to patients as needed.
  4. Completes required documentation, to include individual service plans (ISP), crisis plans, quarterly reviews and eligibility assessments, in a timely and thorough manner, according to established standards and maintains standards of productivity established for the position.
  5. Participates actively in supervision, Team Meetings, patient centered conferences and ISP planning; makes contributions and applies what is learned effectively.
  6.    Participates in team activities and promotes supports for program initiatives.
  7. Introduces patients to resources in the community regularly, utilizing resource information to the best advantage for the patient. 
  8. Assists patients in applying for and/or maintaining financial benefits for which they are eligible. 
  9. Monitors medications prescribed to patients in the program as required and monitors compliance, provides education on prescribed medication and remains familiar with medications prescribed.
  10. Establishes a staff development plan with Supervisor and makes a concerted effort to seek training into areas identified in the plan.
  11. Provides community education informally as appropriate and participates in more formal educational presentations as requested.
  12. May be required to transport patients in their personal vehicle.
  13. May perform other related job duties as assigned by the Director of the Community Support Program or the Director of the SUD Program.
  14. Attendance is a requirement of this position. 

 

Required Skills and Abilities:

  1. Is able to establish a rapport with patients.  Treats patients with dignity and respect and takes a patient centered approach treatment, as noted through supervision, team participation, and feedback from patients and staff.  Is able to build a relationship of trust, as observed by patients, supervisor, and team.
  2. Provides input into program issues and shares/receives feedback in a healthy, constructive atmosphere.
  3. Has general knowledge about medications and diagnosis as well as a working knowledge of issues, rehabilitation and supportive living concepts associated with adult patients and uses these appropriately with patients.
  4. Has working knowledge of crisis intervention techniques and counseling principles and remains current in EBP’s provided to patients.
  5. With the assistance of the supervisor, is able to organize and prioritize a workload so that rehabilitative and administrative duties are completed on time an in accordance with productivity and quality standards.
  6. Follows through with responsibilities to the satisfaction of contact persons and/or supervisor.
  7. Learns the philosophy and operating procedures of Genesis and applies them effectively both within and outside the agency.
  8. Represents the patients/program/agency constructively both within and outside the agency.
  9. Maintains general knowledge of community resources and presents these to patients/supervisor/team as appropriate.
  10. Maintains a working knowledge of the benefits systems and utilizes this information to the best advantage of the patients.
  11. Ability to use computers and word processing equipment.
  12. Ability to express or exchange ideas by means of the spoken word and to receive detailed information through oral communication.  Primary functions include activities in which verbal communication with patients and with others is key to job effectiveness.
  13. Visual acuity must meet a minimum standard to work with preparing, reading and analyzing data and reports, work with computer terminals, etc.

 

QUALIFICATIONS:

Education:  Bachelor degree in Social Work, Psychology, Counseling or other approved credentials as specified in He-M426-11 and He-M426-13.

Experience:  1 to 2 years’ experience in mental health and substance sue disorder preferred.

License/Certifications:  Must be a Certified Recovery Support Worker (CRSW) or eligible to become a CRSW in NH.  Valid NH driver’s license.  Able to pass LRMHC criminal records and motor vehicle screening.   Must maintain personal auto insurance at a level commensurate with agency requirements.

 

WORK SCHEDULE:

Generally works Monday through Friday with occasional requirements to cover emergencies after hours.
