Position Summary:
The RN Start of Care Home Care Nurse is a key member of our Home Care team, responsible for initiating care for new patients in their homes. This role involves conducting comprehensive assessments, developing individualized care plans, and coordinating care with interdisciplinary teams. The RN Start of Care Nurse ensures that patients receive high-quality, personalized care from the very beginning of their home health journey. The Start of Care Nurse will also perform the Resumption of Care visits. This is a full-time Mon-Fri position with some on-call responsibility. This position offers excellent salary and benefit packages.
Primary Duties and Responsibilities
- Conduct Start of Care Assessments: Perform thorough initial assessments of new patients in their homes, including physical, psychosocial, and environmental evaluations.
- Care Planning: Develop and implement personalized care plans, incorporating input from patients, families, and other healthcare professionals.
- Patient Education: Provide education to patients and their families on care plans, medication management, wound care, and disease management.
- Collaboration: Work closely with interdisciplinary teams to coordinate comprehensive care for patients.
- Documentation: Accurately document patient interactions, assessments, care plans, and progress notes in accordance with agency and regulatory standards.
- Compliance: Ensure all care provided complies with state and federal regulations and Lakes Region VNA's policies.
- Quality Improvement: Participate in quality improvement initiatives to enhance patient care and outcomes.
- Supervisory Visits: Conduct supervisory visits as needed to assist in education and in the field training for the nursing team.
Communication:
- Communicate effectively with physicians and community health resources to coordinate care plans.
- Ensure that arrangements for equipment and other necessary items and services are available.
- Promote health and independence across the lifespan through our skilled services and education of patients, families, and caregivers.
Qualifications, Skills, and Competencies:
- Graduate of an accredited school of nursing.
- 1-2 years of recent patient centered care/nursing experience.
- Current state licensure, CPR certification, and a valid driver’s license.
- Bachelor's degree in nursing with one year of home health care experience preferred.
- Strong observation, communication, and problem-solving skills.
- Physical capability to lift patients and equipment, with considerable walking, standing, reaching, bending, and kneeling.

Physical Requirements
- Prolonged or considerable walking or standing.
- Ability to lift, position, or transfer patients (up to 50 lbs.).
- Frequently reaching, stooping, bending, kneeling, or crouching.
- Visual acuity and hearing to perform required nursing skills.
Supervisory Responsibilities
- Supervise and coordinate care with RNs, LPNs and LNAs.
