The Licensed Nurse Assistant is assigned to clients by the Clinical Director, Case Manager, LNA Scheduler/Coordinator or Qualified Clinician. Written instructions for patient care are prepared by the nurse or therapist as appropriate and entered into a care plan to be documented within the medical record. Direct and non-direct supervision by the appropriate professional staff member will be done regularly to assure competent care and compliance with CMS guidelines.
We offer Full-Time, Part-Time, Baylor, and Per Diem hours with excellent salary and benefit packages. While home care experience is beneficial, we provide training for the right individuals. Strong clinical support and educational resources are also available.

Primary Duties And Responsibilities
Patient Care:
- Provides quality, competent, safe, delivery of home health care services by implementing current standards of nursing practice.
- Gives personal care of patients, including bath, mouth care, simple dressings, shampooing hair, nail cutting, etc.
- Assists with ambulation and exercise.
- Performs homemaking services including planning and serving meals, light cleaning, laundry, simple record-keeping, and teaching of household skills.
- Provides emotional stimulation and support.
- Reports physical, social, and emotional problems to the case manager or supervisor.
- Completes documentation within the medical record at the time of the visit.
- Complies with agency’s Policy Manual, Personnel Manual, and Procedure Manual.
- LNA will possess appropriate communication skills.
- Completes documentation within the medical record at the time of the visit.
- A current New Hampshire Home Health Aide license is required.
- Must have an interest in and cheerful attitude toward caring for the sick at home.
- Elevated level of personal hygiene and health habits. A clean neat appearance is essential.
- Good physical and emotional health.
- Ability to function as a team member under the supervision of a professional nurse.
- Must have an available automobile, current driver’s license in the State of NH and verification of appropriate insurance.
