Verbalizes understanding regarding polices on sick calls and absenteeism

Manage Patient Care population and assigned staff on the unit.
Demonstrate ability in communication with other dept. diagnostic and lab
Demonstrate knowledge in cardiac rehab program,
Transitions in Care/ CHF
Completes:
Hand off communication
Admission assessments with documentation
Medication reconciliation
Braden Scale
Pain Assessment
Risk for Fall Assessment
Wandering/Elopement Assessment
Weight/Height Assessment
Vital Signs
Review of admission assessment
Knowledge and follow up of consults
Knowledge in reading labs,& diagnostic and communicating with MD with abnormal /critical values/
Updates and renews RCP as need arises
Follows infection control/standard precautions, at
all times:
Demonstrates proper hand washing procedure
Demonstrates proper disposal of sharps
Appropriate use of personal protective equipment
(PPE)
Follows aseptic dressing procedure
Demonstrate proper procedure in IV insertion, blood drawing, f/c care, ostomy, nephrostomy
Oversee Maintenance of overall cleanliness of resident’s surroundings.
Maintains environmental safety on units
Shows respect of residents’ rights in the overall care.
Demonstrate competency in nursing assessment of acutely ill patient
Demonstrate knowledge in emergency (RRT)
Performing CPR
Initiating transfer paper works/ SBAR
Communicating with MD
Follows Lawrence Nursing Care Center
procedures such as:
Narcotic Count
Transcription of medication orders
Oversee treatment and medication orders with treatment and medication records.
Oversee Medication administration with right drug, right dose, via right route, at the right time to the right resident
Oversee Observation/documentation of response to medication(s)/treatment(s)
Familiarizes with pharmacy pickup procedures
Oversee proper care of resident with g-tube feeding.
Follows procedures related to skin integrity
program such as:
Completes risk assessment as per procedures
Promotes staff compliance to skin care protocols
Reports changes in nutrition status to nutritionist
Communicates pertinent reports regarding skin integrity to nurse practitioner/MD
Follows procedures related to tracheostomy
care such as:
Able to suction patients/trach patients
Maintain aseptic technique
Care of tracheostomy
Follows fall prevention procedures.
Demonstrate knowledge in completion and initiating incident report
Reports to the supervisor and MD (whenever appropriate) any observed change(s) in the residents’ condition.
Documents daily ADL changes as per Medicare guidelines.
Documents using focus charting for episodic events.
Hands off reporting using SBAR communication tool.
Participates on unit quality improvement activities.
Participates in care planning
Initiates CCP and completes CCP document
