Position Summary:
The Social Services Coordinator plays a vital role in supporting the emotional, social, and psychological well-being of residents in the Skilled Nursing Facility. This position is responsible for planning, developing, and directing social services in accordance with regulatory requirements and Legacy Pointes standards of care. The coordinator ensures residents psychosocial needs are addressed throughout their stay, from admission through discharge, and serves as a liaison between residents, families, staff, and outside agencies. This role leads discharge planning efforts, facilitates resident rights education during employee orientation, and oversees the resolution of grievances and concerns in a timely and compassionate manner. The Social Services Coordinator contributes to interdisciplinary care planning, ensures accurate and compliant documentation, and promotes a culture of dignity, autonomy, and person-centered care. Duties and responsibilities will be completed while demonstrating behaviors that align with our communitys service excellence philosophy and core values.

Essential Duties and Responsibilities:
  • Plan, develop, organize, implement, and evaluate the social services program to meet the ongoing needs of residents in the Skilled Nursing Facility.
  • Conduct initial and ongoing psychosocial assessments for all residents and ensure documentation is accurate and timely.
  • Serve as the primary contact for discharge planning; coordinate post-discharge services such as home health, outpatient rehab, or long-term care placement, and ensure residents and families understand their discharge instructions and insurance coverage.
  • Participate in Medicare coverage discussions with the interdisciplinary team to support timely and appropriate discharge planning.
  • Attend daily stand-up and care planning meetings to provide psychosocial input, address concerns, and communicate resident and family needs.
  • Act as the facilitys primary point of contact for grievances and complaints related to care, services, or interpersonal issues; conduct timely investigations and collaborate with leadership to develop and implement appropriate resolution plans.
  • Document grievances, resolutions, and trends to support QAPI and regulatory compliance.
  • Provide in-service education during employee orientation and ongoing training, specifically related to Resident Rights and Abuse, Neglect, and Exploitation prevention and reporting procedures.
  • Maintain current knowledge of Florida regulations, CMS guidelines, and ethical standards surrounding elder care and resident protections.
  • Monitor resident emotional well-being and intervene or coordinate behavioral health support when necessary.
  • Educate residents and responsible parties about Medicare coverage, available community resources, legal documents such as advance directives, and long-term planning.
  • Support residents and families during difficult transitions, including end-of-life care, palliative services, and bereavement.
  • Partner with nursing and administrative teams to advocate for a person-centered, inclusive culture that prioritizes dignity, autonomy, and hospitality.
  • Lead social services-related preparation for surveys and inspections, including ensuring all documentation is compliant and readily available.
  • Maintain all required documentation in the Electronic Medical Record (EMR) system and ensure entries are timely, accurate, and regulatory-compliant.
  • Participate in QA/QAPI meetings, care plan reviews, and policy updates related to psychosocial services and resident advocacy.
  • Serve as the liaison between the facility and external resources (e.g., protective services, ombudsman, legal aid) when appropriate.
  • Assist in training, mentoring, and supervising social services team members, if applicable.

Educational Requirements and Experience:
  • Bachelors degree in Social Work or a related human services field (e.g., sociology, psychology, gerontology, rehabilitation counseling).
  • Minimum of two (2) years of experience in a healthcare or long-term care setting.
  • Supervisory experience preferred.
  • Knowledge of Medicare and private insurance coverage related to skilled nursing facility services, discharge planning, and care transitions.
  • Familiarity with MDS documentation and interdisciplinary care planning is a plus.

Knowledge, Skills, and Abilities:

Language Ability:
  • Ability to communicate effectively in writing and verbally, speaking the primary language of the residents.
  • Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
  • Ability to write routine reports and correspondence.
  • Ability to speak effectively before groups of customers or employees of organization.
Mathematical Skills:
  • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
  • Ability to compute rate, ratio, and percent, and to draw and interpret graphs.

Cognitive Demands:

  • Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form.
  • Ability to deal with problems involving several concrete variables in standardized situations.
Computer Skills:
  • Proficiency in Microsoft Office and electronic health records.
  • Ability to use internet-based resources and internal systems for communication and documentation.

  • Ability to work weekends, holidays, or evening shifts as needed.
