Job Overview:
We are seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will be responsible for providing support and assistance to individuals and families in need. Your role will involve assessing client needs, developing and implementing care plans, and connecting clients with appropriate resources and services.
Duties:
- Conduct comprehensive assessments of client needs, including physical, emotional, and social factors
- Develop individualized care plans based on assessment findings
- Provide ongoing case management services to clients, including regular check-ins and monitoring progress
- Collaborate with other service providers and community organizations to coordinate care and ensure clients receive necessary support
- Assist clients in accessing resources such as housing assistance, healthcare services, employment opportunities, and educational programs
- Advocate for clients' rights and needs within the community
- Provide crisis intervention as needed
- Maintain accurate and up-to-date case records
Experience:
To be successful in this role, you should have the following qualifications and experience:
- Associate's degree in social work or a related field (Bachelor's degree preferred)
- Previous experience in case management or a related field
- Knowledge of social service programs and resources, such as Section 8 housing, workforce development programs, addiction counseling services, childcare assistance, etc.
- Familiarity with child welfare systems and regulations
- Strong communication skills, both written and verbal
- Ability to build rapport with diverse populations
- Proficiency in motivational interviewing techniques
- Excellent organizational skills and ability to manage multiple cases simultaneously
- Ability to work independently and as part of a team
