LeRoy Village Green RHCF is searching for a Social Worker to become part of our facility's compassionate care-giving team. We are a 140-bed skilled nursing facility located in the scenic village of LeRoy in Genesee County, New York.
Social Worker Requirements:
- BSW Certification or Master's Degree in Social Work
- Previous Short Term/Sub-Acute Experience
Social Worker Duties:
- Completing MDSs
- Maintaining an open line of communication with residents and families
- Attending care plan meetings
- Discharge planning
