Job Summary and Qualifications
Our Case Management Assistant (CMA) will work with oversight and direction from the RN Case Manager or Social Worker to assist with facilitating patient discharges requiring post-acute placement or services, and will identify and report barriers to discharges. This role will provide administrative/clerical support to all Case Management functions.
- You will be responsible for coordinating with the RN Case Manager or Social Worker for support needed and will communicate updates or patient questions or concerns throughout your shift.
- You will be responsible for providing clerical support in multidisciplinary rounds, including updating NATE Tempo at the direction of the team.
- You will make referrals through the web-based referral system to the specific post-acute care (PAC) providers, which also includes sending the appropriate packet of information from the medical record.
- You will monitor patients with PAC referrals and intervene as needed to expedite discharges.
- You will be responsible for prioritizing workload to ensure a timely contact and intervention with PAC providers.
- You will monitor Medicare patients planned discharge dates and deliver the Important Medical Message (IMM) to the patient per facility policy.
- You will deliver the Beneficiary Notice to patients identified as being in the BPCI-A program (Bundle Payment hospitals only).
- You will obtain physician signatures as need on required Case Management documents and forms.
- You will set up transportation and send information to the Transfer Center and generate discharge packets.
- You will provide any all additional requests made by the CM team to help support them within your scope of practice.
What qualifications you will need:
- Associates Degree is preferred.
- Previous receptionist or secretarial experience that includes efficient computer skills.
- One year or more of previous healthcare experience is required.
- Knowledge of medical terminology is required.
- EMT, CNA, or other certifications in the healthcare field is preferred.
