Duties:

As a Case Manager at a Medical Referral Company, you will be responsible for providing comprehensive patient coordinating services to ensure the best possible outcomes. Your duties will include:

- Conducting patient assessments to determine their needs and develop individualized care plans.

- Collaborating with healthcare professionals to coordinate and facilitate patient care.

- Monitoring patient progress and adjusting care plans as needed.

- Providing patient education and support to promote self-management and adherence to treatment plans.

- Maintaining accurate and up-to-date documentation of patient interactions and interventions in our custom software.

- Strong patient care skills with a focus on delivering high-quality, compassionate care.

- Proficiency in conducting comprehensive patient assessments.

- Excellent interpersonal and communication skills to effectively collaborate with healthcare professionals, patients, and their families.

- Solid understanding of medical terminology and physiology to accurately interpret medical records and communicate with healthcare providers.

- Ability to work independently, prioritize tasks, and manage a caseload effectively.

Pay: $20.00 - $24.00 per hour
