Position Title: Community Based Service Coordinator
Reports to: Director of Clinical Operations
Job Status: Full-Time Position with benefits, 401k, and PTO
Pay: Base salary with bonuses and incentives
- Starts at $34,000 – diploma or GED with 5 years of direct service experience
- Starts at $36,500 – bachelor’s degree in human services field
- Starts at $42,000 – master’s degree in human services or counseling field
POSITION SUMMARY
The Community Based Service Coordinator will coordinate and oversee the clinical and support service processes of the agency, to include providing support to the direct service team. This position encompasses administrative coordination responsibilities and direct services in the community applicable to education level.
EDUCATION AND EXPERIENCE
- One of the below education levels: High school diploma or GED with minimum of 5 years direct service experience or Bachelor’s degree in human services field or Master’s degree in a Human Services or Counseling (and eligible for licensure) related field
- Previous experience in providing direct services in the community
- Experience providing a combination of both direct services and administrative support in a therapeutic environment
- Familiarity with eCase notes is a plus
ESSENTIAL DUTIES AND RESPONSIBILITIES
Responsibilities will include but are not limited to:
- Coordinate services and provide support to the direct service team to also include assigning cases and requesting services
- Monitor service referrals to ensure that services are completed efficiently and timely
- Address any concerns or complaints concerning the direct service team to ensure that policies and procedures are followed
- Recommend corrective action for direct service team members as appropriate and create performance improvement plans when needed
- Identify and recommend solutions to direct service team and agency to resolve issues or problems
- Ensure that team members have access to resources and screening tools needed to complete referred services
- Conduct periodic support meetings with team, when needed
- Build relationships and collaborate with case managers, contractors, and community partners to ensure effective treatment and case collaboration
- Assist with interviews and also conduct orientation and training on agency policies and procedures
- Create and facilitate trainings to support the agency service goals
- Hold a clinical (Therapy and/or Assessments) or support (BA, Transport, Drug Screens) service caseload (Subject to education level)
- Document case notes in the electronic health record system (eCase Notes)
- Maintain excellent communication and effective working relationships with other agency staff
- Manage and implement agency processes to ensure a standard of care is met
- Maintain all applicable certifications and licensure including CPR certification
- Attend agency meetings and trainings pertinent to the role and relay relevant information to direct service team

WORK ENVIRONMENT
- In office position with some community engagement
- Based on a 40-hour work week with at least 1 evening day
- Some weekend work may be required
- Interaction with a diverse population including team members, providers, clients, etc.
