What are the responsibilities and job description for the Non-Clinical Case Management (RN/LPN) position at LifeHealth, LLC?

LifeHealth, a federal government contractor, is seeking a Nurse (RN or LPN) to provide a variety of non-clinical case management activities in support of members of the California Army National Guard. 

 

Nurse Case Manager responsibilities Include (full job description provided at time of interview):

  • Shall track medical status of all Soldiers.
  • Request from the COR, profile exams/evaluations and audiology exams.
  • Track, schedule and follow up on automated referrals indicated in MEDCHART.
  • Submit case processing, medical documentation, and profile concerns to MRO.
  • Provide case manager services at MRE events as determined by the needs of the state and the individual units.
  • Manage Soldier’s care from the identification of the condition to when the Soldier is either:

    • Considered medically ready
    • Administratively separated from the military
  • Other duties as assigned.

Your skills/experience/job requirements:

  • Shall be at least a Licensed Vocational Nurse / Licensed Practical Nurse (LVN/LPN) or Registered Nurse (RN) with an active state license in good standing in the state of CA.
  • Minimum of 3 years adult medicine clinical practice experience.
  • Utilization review or disease management experience or certification in case management is desirable.
  • Must be computer literate.
  • Must be US Citizen
  • Must be able to obtain a National Agency Check with Inquiries (NACI) with Federal Bureau of Investigation (FBI) fingerprint check.

  • 8 hour workday Monday to Friday, with holidays off. (Travel may be required for SRP and PHA events)
