Case Manager Job Description
Essential Duties & Responsibilities
- Develop a comprehensive aftercare plan for each client.
- Assist clients in developing educational and vocational goals.
- Assist clients with any legal/court issues
- Conduct and document weekly client assessments.
- Facilitate group counseling sessions as assigned by the program director.
- Manage a caseload of up to six clients and facilitate individual case manage sessions twice per week.
- Meet with clients individually at least twice a week for case management sessions and chmical dependency sessions.
- Document all group and individual sessions following DHCS and JCAHO standards.
- Assist the treatment team with any other assigned tasks
Job Requirements
- Minimum of a high school diploma or GED
- Current counseling certification through CCAPP or CADTP.
- Minimum of 2 years experience as a counselor in an AOD program.
Skill Requirements
- Strong documentation skills
- Experience in KIPU is a plus
License
- Valid driver's license (required)
