Adult Care Specialist

Lighthouse Youth & Family Services is looking for dedicated Adult Care Specialist for our Transitions to Independence Program, a 4-bed transitional housing program for 18- to 24-year-old, all gender, young adults. Our goal is to transition young adults to permanency as quickly as possible through coordinated, tailored, intensive services for each young adults. We have a part time 2nd shift available.
Starting pay: $16.92 an hour
Essential Functions:
- Implementation of program goals and objectives to promote a strengths-based, youth-centered approach to assisting residents in achieving individual goals and ultimately transitioning to a less restrictive setting.
- Responsible for supervising youth while maintaining a safe and productive environment at all times.
- Provide individual and group skill training, supervision and support in areas such as self-care, hygiene, community resources, and other life skills as directed by the mental health individualized service plans.
- Provide therapeutic interventions with residents as required and directed by individualized service plans and complete all subsequent documentation.
- Consistently utilizes trauma informed language and a strength-based approach to assist our young adults to reach their fullest potential
- Prepare meals, facilitating group sessions, and outings, providing security, observation, housekeeping, preparing medication completing intakes, shift paperwork and other duties as assigned.
- Complete documentation in an electronic health record (EHR) as required using CareLogic software.
- Assist team by completing paperwork related to program outcomes. Document any significant events that occur during the shift in a daily log, service ticket and/or progress note.
- Coordinate with oncoming shift regarding resident concerns, building/program needs, and any relevant issues to ensure the smooth operation of the program.
- Provide direct supervision of youth and facility throughout the work shift.
- Supervise activities, chores, hygiene, and bedtimes times by following program schedule.
Qualifications:
- High School diploma or GED required.
- Good written and verbal communication skills. Bilingual and/or Sign Language proficiency a plus.
- Minimum 2 years work experience in serving at risk youth or in a residential program preferred.
