Lighthouse Youth & Family Services has one Full Time Shelter Youth Outreach Case Manager position available at Lighthouse Sheakley Center for Youth. The scheduled hours for this position are Monday through Friday first shift.
Essential Functions:
- Candidate will be responsible for being a part of the larger Street Outreach team services who work on the streets encountering and engaging homeless young adults.
- Candidate will provide intensive case management to homeless youth 18-24 who are experiencing homelessness.
- Duties will consist of assisting in harm reduction, in-depth assessments, helping to secure stable housing or shelter, employment readiness, monitoring mental health/substance abuse concerns, linking to community resources and aftercare follow-up. Timely and accurate data entry and monitoring client outcomes is required. Crisis de-escalation and group facilitation skills are a must.
- Willing and able to work a flexible schedule and be on feet for extended periods of time.
- Able to work in a variety of environmental conditions as part of outreach efforts.
Qualifications:
- Bachelor’s Degree required, BSW preferred
- Experience working with homeless or at risk youth or working in a social service agency is preferred
- Prefer candidates who have experience working with harm reduction principles, particularly with regard to sexual risk, substance abuse, mental illness and LGTBQ.
- Must have excellent documentation skills, car, and good driving record
