Certified Diabetes Care and Education Specialist
Lincoln Community Health Center, Inc., a federally qualified community Health Center (FQHC) located in Durham, NC is recruiting for a Certified Diabetes Care and Education Specialist. Durham is known as the Bull City, the City of Medicine, the Tastiest Town in the South, and the Startup Capital of the South. It’s a two-hour drive from the beach, a three-hour drive from the Blue Ridge Mountains, and is North Carolina’s fourth largest city by population. Along the East Coast, Durham is located conveniently along I-85. For those outside of driving distance, travel to and from is made easy as the City and County of Durham co-own the top-ranked Raleigh-Durham International Airport that services more than 45 nonstop destinations.
Position Summary: The Certified Diabetes Care and Education Specialist (CDCES) is a key member of the health care team and works closely with providers, nursing staff, management, and other population health staff. The CDCES possesses clinical expertise in care related to diabetes management and education and other chronic diseases.
Knowledge, Skills, and Abilities: The Certified Diabetes Care and Education Specialist (CDCES) possesses clinical expertise in care related to diabetes management and education and other chronic diseases. Good written and verbal communication and materials development skills. Desire and ability to work collaboratively, as a member of a team. Strong positive Customer Service Skills. Strong processing abilities. Strong problem-solving and communication. Ability to adapt education approach based on patient goals, learning preferences and abilities, educational attainment, socioeconomic status, culture, English proficiency, and age of clients.
Education and Experience: The position requires a bachelor's degree in an appropriate discipline. Licensed Dietitian/Nutritionist or licensed to practice as a Registered Nurse in the state of North Carolina with Certified Diabetes Care and Education Specialist (CDCES) credentialing. Spanish language fluency is a plus.
