Description

III.    Essential Job Functions

  1. Read, write, speak, hear, reason and remember.
  2. Climb stairs, stoop, kneel, crouch, reach, stand, walk, push, pull and lift light weight (24 lbs.).
  3. To observe and assess patients’ condition, implement the plan of care, and communicate findings to DOPS and primary MD as appropriate.
  4. Possess a valid New York State driver’s license and have access to an automobile available for work use.
  5. As a professional working independently in the community, the individual must possess self direction, excellent interpersonal communication skills, organizational skills, and the ability to problem solve
  6. To perform all job duties and responsibilities as outlined below in IV.

 

 

 IV.    Job Duties and Responsibilities

 

The Specialty Home Care RN is supervised by the Director of Patient Services (DPS).  The job duties and responsibilities are to be performed while observing the following standards: job standard (knowledge, ability, skills), technical skill, quality, quantity, judgement/problem solving, decision making, and initiative.  The responsibilities include, but are not limited to the following:

 

  1. Perform assessments of physical, emotional, psychosocial and environmental needs of patients.  Report signs and symptoms signaling a change in patient condition to the primary C and/or the physician.
  2. Establish a comprehensive and coordinated Plan of Care with patient/caregiver/physicians.
  3. Timely implementation and evaluation of appropriateness of Plan of Care. 
  4. Assist with making appropriate revisions to the Plan of Care on an ongoing basis, with communication with the DPS, other professionals and para professionals and caregivers involved in SCHC patients’ care.
  5. Maintain current, complete and accurate clinical records, submitted in a timely manner, as per SCHC policy and regulatory guidelines.
  6. Provide supervision and ongoing education to HHA’s and PCA’s who provide services to SCHC patients.
  7. Participate in case conferences.
  8. Receive assignments from a supervisor and complete all assigned visits.
  9. Provide education to patient/family/caregiver concerning knowledge deficits or needs regarding the patient’s condition.
  10. Perform other related duties and responsibilities as required by their manager.

 

 

V.        General Duties and Responsibilities

  1. Participate in all department meetings, mandated inservices, and assigned committees.
  2. Maintain a positive and professional relationship with residents, registrants, family members, legal representatives and visitors.
  3. Maintain a positive and professional relationship with all personnel, including supervisors, co-workers and volunteers.
  4. Follow all safety regulations and precautions at all times; use protective devices as required.
  5. Wear and maintain proper uniform/clothing as established in the uniform dress code policy, including name tag identification.  Maintain acceptable personal hygiene.
  6. Maintain confidentiality and resident/patient/registrant rights.
  7. Maintain an acceptable attendance rate.
  8. Follow all laws, rules and regulations of the Federal and NYS Health Codes in regard to the department and overall organization operations.

Qualifications


II.      Qualifications

 

Education and Training

  • Registered Professional Nurse (RN) with current licensure and registration in New York State.
  • Graduate from an accredited nursing program, holding one of the following qualifications:
    1. A Baccalaureate Degree in Nursing from an approved program, or
    2. A diploma from a New York State accredited RN Hospital Diploma Program.

 

Experience

  • Previous experience as a Home Care Nurse is required.
  • At least one year of medical/surgical or clinical assessment experience is required.
  • Minimum of 3-5 years of nursing experience, including expertise in wound care and infusion therapies.
  • Experience in Adult Oncology Nursing and Chemotherapy is preferred.

 

Character

  • Must demonstrate good moral character, patience, and compassion for the elderly, chronically ill, and disabled.
