POSITION STANDARDS:
The Registered Nurse is to coordinate team-based care to provide health services to individuals, through effective partnerships with patients, their caregivers/families, community resources, and their physician. Facilitates a “shared goal model” within and across settings to achieve coordinated high-quality care that is patient and family centered.
SUMMARY AND ESSENTIAL FUNCTIONS:
- Promote the mission, vision, and values of the organization
- Recognizes and responds to opportunities for improvement.
- Assess patient prior to and post pain procedures
- Provide effect communication to improve health literacy.
- Obtain vitals and provide accurate patient assessments on patients
- Monitor patient’s adherence to plan of care and progress toward goals in a timely fashion, facilitate changes as needed.
- Be able to provide triage support for patients within the practice.
- Facilitate patient access to appropriate medical and specialty providers as well as other care coordination team support specialists
- Cultivate and support care with timely communication, inquiry, follow up, and integration of information into the care plan regarding transitions-in-care and referrals.
- Serve as a point of contact, advocate, and informational resource for patient, family, care team, payers, and community resources and facilitate and attend meetings between patient, families, care team, payers, and community resources, as needed
- Ensure effective tracking of test results, medication management, and adherence to follow-up appointments.
- Collect data and write brief reports to meet evaluation needs of the program
- Urgent Diagnostic Imaging prior authorizations
- Provider request follow up
- Responsible for daily assignments, monitoring staff needs, management of acute needs of office staff
- MA support
- Quality issues/patient related
- Assist with surgical and procedure scheduling and coordination
- Accurate procedure documentation and care to the Registered Nurse standard
REPORTS TO: Practice Manager
SUPERVISES: None
INTERNAL AND EXTERNAL CONTACTS:
Patients, office staff, providers, hospital personnel, pharmaceutical representatives, nursing home personnel |
QUALIFICATIONS:
Experience/Specialized Skills: Currently licensed as a Registered Nurse in the State of New Hampshire Graduate of an accredited school of nursing Bachelor of Science preferred 1-2 years previous clinical nursing experience preferred
Required Education/Course(s)/Training: Currently licensed as a Registered Nurse in the State of New Hampshire Graduate of an accredited school of nursing Bachelor of Science preferred
Preferred Certification/Registration: BLS required ACLS Required
