St. Luke Lutheran Community is currently seeking a dedicated and experienced Licensed Social Worker (LSW) to join our team. This role will support both our North Canton and Portage Lakes campuses, with the majority of time spent at the North Canton location.
What You’ll Do:
- Coordinate social services across departments through clear, collaborative communication
- Conduct interviews with residents to gather personal history and guide care planning
- Offer counseling and referral services to help meet psychosocial and financial needs
- Share insights on residents' social and emotional needs with the care team
- Support room transitions and help residents adjust as needed
- Manage discharge planning and coordination
- Maintain and follow standard operating procedures for the department
- Active and valid LSW license in the State of Ohio
- Previous experience in long-term care or geriatric social work
- Ability to follow instructions, accept feedback, and work independently when necessary
- Strong communication skills with the ability to build positive working relationships
- Knowledge of medical and nursing practices, regulations, and long-term care standards
