We are seeking a dedicated Dietitian to join our team at Living Branches. In this role, you will be responsible for developing and implementing nutritional care plans that enhance the health and well-being of our residents. You will also have the opportunity to teach principles of food and nutrition to residents or counsel residents with various needs. Your expertise will contribute significantly to our mission of providing life-enriching services.
Nutrition Care Coordinator Requirements:
- Assess residents' nutritional needs and develop individualized care plans
- Conduct diagnostic evaluations to monitor residents' health status
- Collaborate with healthcare teams to ensure comprehensive dietary management
- Supervise dietary department staff to maintain high standards of food service
- Provide health coaching and education on nutrition and wellness to residents
- Completes MDS nutrition screening and assessment
- Assists with implementation of all regular and therapeutic menus and modified spread sheets
- Attends care plan meetings and ensures proper documentation and follow through
- Stay updated on best practices in dietetics and geriatric nutrition
- Bachelor's degree in Dietetics or related field; Registered Dietitian license required
- Experience in long-term care or geriatrics is highly desirable
- Strong knowledge of care plans and dietary management systems
- Excellent communication skills for effective interaction with residents and staff
- Ability to supervise and lead a team in a dietary setting
