What are the responsibilities and job description for the MDS COORDINATOR position at Los Palos Post Acute Care Center?
Join our nursing team as an MDS Coordinator! In this role, you will be involved in the Patient Assessment Process through the completion of pertinent items on the Minimum Data Set (MDS) and the accompanying Care Area Assessments (CAAs). You will also participate in developing individualized patient Care Plans, identifying the needs of the patient and projected outcomes as required by Federal and State regulations.
